We suspect that differences in apparent diffusion coefficient (ADC) between two models of ischemia (middle cerebral arterial occlusion, MCAO, and photo-thrombosis, PT) could be detected with MRI, using MAP2 IR to confirm the extent of the infarct. Nine Sprague-Dawley rats under isoflurane anesthesia and controlled ventilation were subjected to either MCAO combined with bilateral common carotid artery occlusion (n=6) or cortical PT (n=3). MRI was used to generate ADC trace (ADCtr), pefusion (spin-labeling, CBF), and T1 images. After 3.5±0.3 hr (mean±sem), the brain was immersion fixed and processed for MAP2 IR to HM2 (Sigma) antibody. The stained section in the same coronal plane as the MR images was digitized and aligned with the ADCtr, CBF, and T1 images using the AIS/C imaging system. Four regions-of-interest were chosen: the ischemic core (IC), cortex immediately adjacent (ischemic border, IB), and homologous regions in the opposite hemisphere including normal cortex (NC), guided by the lesion area from the immuno-stained section. PaCO2 did not differ between the two groups (42.5±3.5 mmHg). In the IB, the ratio of ADC (IB/NC) in the MCAO group was depressed (72%±6% p<0.02) compared to the PT group, whereas the drop in CBF was not significant. CBF differed in NC (134±10 and 199±20 ml/100g/min, p<0.01, MCAO and PT, respectively), and T1 differed in IC (1.7±0.2 and 2.2±0.1 sec, p<0.02). However, only T1 differed when expressed as a ratio, IC/NC, between MCAO and PT (see Fig., T1 on right axis). No other differences were noted. ADC was depressed in the border zone in the MCAO group, but not the PT group, suggesting marginal perfusion, but CBF did not drop significantly. The increased T1 in the PT group in IC is no doubt related to the different character of edema. In the IC ADC did not discriminate between the types of ischemia at 3.5 hr after onset, suggesting that the mixture of vasogenic and cytotoxic edema might yield the same diffusion characteristics in the two groups.
